59-year-old female (J.K.) from Columbus LTAC with a prior stay at Lehigh Valley Cedar Crest Hospital. Patient admitted to Cedar Crest Post Acute with Acute Respiratory Failure, Diabetes and CKD Stage 3. Patient is admitted with a Trach Tube S/P Vocal Cord Paralysis.
Medication Management – Atorvastatin, Insulin, Oxycodone
Monitor Labs and Diagnostic Testing – CBC/ Diff, BMP
Treat Infection – Pneumonia
Maintain Pain Free – Right Knee Osteoarthritis
Maintain Patent Airway; Secure ENT assisted Trach Change – Goal Met
Maintain adequate oxygenation – Wean Oxygen as tolerated – Goal Met
Secretion Clearance and Expansion Therapy – Ongoing
Improved Quality of Life through with Speaking trials – Ongoing
Our Full Time Respiratory Therapist supported Lehigh Valley ENT, Dr. Mausumi Syamal for a trach tube change.
Corticosteroid Injections provided onsite by Dr. Chiou are assisting in patient plan of care for continued improvement.
Patient continues to improve and gain strength needed to return home with her husband. The patient is working on Bed mobility and standing tolerance. Patient is determined to return home with her husband and continues to improve each day.
